Sciatica, Sciatic Nerve, Back Pain, Back AcheIt would be very rare to meet an adult who has never had an episode of low back pain.  An unlucky portion of these people may also experience sciatica, which is pain that radiates from the lower back through the hips, buttocks, and leg along the path of the sciatic nerve.  Many times, people turn to medications or other gadgets like support belts and shoe insoles in order to find relief.  By some estimates, people across the U.S. have spent up to $80 in one year for treatment of spinal issues, including lower back pain.

This may come as a surprise, but the pain in your low back may actually be coming from a spinal problem originating in the neck.  Your uppermost two vertebrae in the neck are unique from all of the others.  They are freely movable and do not have interlocking joints like the rest.  This makes them especially prone to misaligning, and when they do, it sets off a cascade of compensations throughout the rest of the spine.  This can not only cause pain and discomfort in the back but also the radiating leg pain of sciatica.  No amount of adjusting to the lower back will provide lasting relief since the main issue is elsewhere in the spine.

How is my low back pain related to my neck?

Your spinal cord sends signals to and from your brain. Your nerves branch off from your brain and spinal cord. Your spine is designed to protect your vital spinal cord. When the top bone in your spine (atlas) is out of alignment, it creates spinal cord tension. This tension leads to the signals becoming distorted. Improper signals can lead to postural imbalance, pain, and muscle spasms. One of the ways an atlas misalignment can create low back pain is through postural imbalance. Muscle tension can increase on one side of the body, causing one hip to be higher than the other. This causes one leg to be slightly shorter than the other.

I am trained in QSM3 and NUCCA, two precise chiropractic techniques. Through the use of these approaches, I am able to gently realign the atlas, reducing pressure within the spinal cord. This leads to proper signals being sent to and from the brain, potentially creating balanced posture. Less muscle tension leads to even hips, possibly reducing low back pain symptoms.

Chung et al. described a case study of a 57-year-old man who experienced sciatica down both legs, along with other health issues. After approximately one month of NUCCA care, his sciatica pain reduced by 90%. After 2 months of care, he experienced complete resolution of his sciatica (2).

Herniated Disc, Sciatica, Sciatic Nerve, Back Pain, Back AcheHow can a person know if they are experiencing pain from a herniated disk? You might have one and not even be aware of it. However, they may also cause you intense pain. One of the symptoms is feeling a tingling sensation as if your arm or leg is asleep. You may also experience weakness in the muscles that are affected by the herniation. It may cause you to be unable to lift something or even to stumble while walking. Another symptom to look for is pain in your arm or leg. Whether if affects your arm or your leg largely depends on where the damaged disk is located.

What Is a Herniated Disc?

The disks located in your vertebrae are protected by a gel-like substance that keeps the bones from grinding together. When a herniation occurs, some of this gel leaks out of place. It then causes irritation to the surrounding nerves and muscles. This makes it easier for the disks to then shift. Sometimes this is referred to as a slipped disk. If this area then becomes inflamed, it can cause severe pain.

What Can Be Done?

At this point, it is important to relieve the pressure being put on the nerves and muscles from the herniated disk. This is done by getting the spine back into its proper position. My low back hurts, how is this related to my neck?

The bones of the spine, your vertebra, are designed to protect your spinal cord.  The spinal cord is the highway on which messages between the body and the brain travel.  Nerves branch off the spinal cord to provide information to all the components of your body.  When the top bone in your spine, the atlas, is misaligned, it creates tension throughout the entire spine from top to bottom.  While it may seem strange to think about, the pain you experience in your low back may be a result of a misalignment in your neck.  This is because of the compensations that happen to correct for the misalignment of the head and neck – the shoulders and hips will compensate leading to postural distortions and muscle imbalances.

To correct the compensations that lead to nagging low back pain, Dr. Brett Gottlieb gently works to realign the atlas vertebra.  When normal upper cervical alignment is restored, this brings ease to rest of the spine.  Tension on the spinal cord is released, posture can return to normal, and balance returns to the muscles.  A gentle adjustment to the neck is sometimes all it takes to clear up stubborn low back pain.

Sciatica, Sciatic Nerve, Back Pain, Back AcheThere is a very simple first step you can take to begin improving your back pain right away. It is so simple that you can do it in your sleep. That is because it has to do with the way you sleep. The position you are sleeping in has a lot to do with how your back feels both when you get up and throughout the day.

What Is the Best Sleeping Position?

First, let’s talk about what position you should not sleep in. Sleeping on your stomach is not good for your posture, back, or neck. Sleeping on your side is better than sleeping on your stomach. If you do have back problems, be sure to put a pillow between your knees when you plan to sleep on your side. This eases any pressure on the lower back. The preferred way to sleep for maximum health benefit is to sleep lying on your back. If this is too uncomfortable because of your back pain, sleeping on your side is acceptable.

What Else can Help?

Regular exercise can help alleviate some back pain. Have you heard the saying: The life not lived is not worth living? This can apply to having exercise as part of your daily routine. Research shows that sitting most of the time can cause lower back pain. However, exercise reduces the chance that you will get back pain and also helps to alleviate it in those that already suffer from it. So, make it a goal to get out and do something fun. For example, take a walk, do some Pilates, go swimming, or take an exercise class. Do what makes you feel good and, in turn, it will make your back feel better.

It is also a good idea to have a support team made up of the people you are around on a regular basis. These people can help you to keep motivated. This is very important to your recovery.

A Gentle Option

Dr. Gottlieb is trained in NUCCA and QSM3. These are gentle chiropractic techniques aimed at reducing atlas misalignments. It is incredibly specific to each individual patient. Through realignment of the atlas, tension on the spinal cord is reduced. This may lead to a reduction in pain, muscle spasms, and postural distortion. The hips may level out after a correction, potentially leading to a reduction of pressure on the low back.

Chung et al. reported a case of a male patient who presented with sciatica. He was found to have an atlas misalignment. While under NUCCA care, he noticed a reduction in his sciatica symptoms. (2)

Sciatica, Sciatic Nerve, Back Pain, Back AcheSciatica, or pain that travels along the path of the sciatic nerve, can range from a minor annoyance to a very severe pain.  The sciatic nerve has its origins in the lower back, and travels through the hip area, buttocks, and down the back each leg (though sciatic nerve pain usually only affects one side of the body).  Sciatica can occur when, for one reason or another, the sciatic nerve becomes compressed or pinched.  This typically occurs as a result of a herniated disc or a bone spur.

Much research has been released lately about the risks associated with sitting too much.  Sitting for long periods of time has been linked with a number of health concerns, including elevated blood pressure, abnormal cholesterol levels, and excess body fat.  Prolonged sitting can also contribute to sciatic pain for a number of reasons.  Sitting in a chair shifts more weight onto the discs of the spine in the lower back, which can aggravate the sciatic nerve.  Sitting for extended periods of time also encourages poor posture in the neck, which in turn affects the spine lower down.  Carrying the head off-center or off-level can create imbalances that can travel down the length of the spine, causing pain and discomfort.
